This Full-Time OT position includes a weekend schedule with flexibility through the week.

Responsibilities




Provides and directs Occupational Therapy services to patients including screening, assessment, treatment, program planning and implementation. Functions under physicians’ orders and adheres to applicable principles and practices of occupational therapy, Brookdale policies and procedures and state regulations.
1.Optimizes the physical abilities and skills of patients by performing or supervising therapy staff in initiating referrals; evaluating and treating appropriate patients; effectively participating in the care planning process; appropriately documenting progress toward identified objectives and communicating patient status and needs to the patient, the patient’s family, the staff and other professionals as appropriate.
2.Establishes individual patient plan of care to optimally address patient-specific goals and to comply with service delivery parameters in accordance with the Resident Assessment Instrument (RAI) Manual, Managed Care Contract or other guidelines specific to long term care. Develops treatment plans by identifying a problem list, including long and short-term goals and methods to achieve identified goals.
3.Coordinates treatment plan with patient, family and other team members. Implements and conducts treatment as outlined in the treatment plan. Monitors patient’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician’s orders. Adapts treatment to meet the individual needs of the patient.
4.Accesses the patient’s record and records results of screenings, physician orders, discharge summaries and other information as required in the Community’s electronic medical record system. Documents results of patient’s assessment, treatment, follow-up and termination of services following standards established by The Center for Medicare and Medicaid Services (CMS), Medicare Administrative Contractor Local Coverage Determinations (MAC LCD) and Brookdale, utilizing electronic medical record software as specified by Brookdale
5.Develops appropriate home program to maintain and enhance the performance of the patient in his/her own environment. Participates in discharge planning. Provides/recommends/fabricates adaptive devices, orthotics or prosthetics. Trains patient, family and appropriate staff in the use or application of the above.
6.Refers patients for treatment to Certified Occupational Therapy Assistants (COTA) by completing evaluation and initial plan of treatment and communicating necessary and sufficient information. Supervises and directs COTA in the implementation of the treatment plan.
7.Reviews the quality and appropriateness of the total services delivered and of individual occupational therapy programs for effectiveness and efficiency, using predetermined criteria such as CareTool and other standardized tests and measures.
8.Participates in patient, family and staff education. Confers formally and informally with other team members in coordinating the total rehabilitation program of the patient. Attend in-service training and facility meetings as required.
9.Submits billing, payroll and expense documentation in an accurate and timely manner, meeting all regulatory guidelines. Assists in the collection of statistics as directed.
10.Complies with all Infection Control, Universal Precautions, and OSHA standards for the healthcare professional. Assists with and participates in Quality Assurance and Performance Improvement Programs as required. Adheres to established confidentiality standards as required by Brookdale concerning patient information.
11.Attends in- service training as well as provides education to members of the patient care team and members of the community.

Qualifications




Education and Experience
Bachelor Degree in Occupational Therapy from an accredited program required. Previous occupational therapy experience preferred; or equivalent combination of education and experience.



Certifications, Licenses, and Other Special Requirements
Successful completion of the National Certification Examination for Registered Occupational Therapist. State license in practicing state.



Management/Decision Making
Uses limited independent judgment to make decisions based on precedents and established guidelines. Solves problems using standard procedures and precedents. Knows when to refer issue to supervisor and when to handle them personally.



Knowledge and Skills
Has a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has a working knowledge of the organization. Basic computer software skills in Microsoft Word and Excel. Excellent verbal and written communication skills required.

General Summary of Position

MNRN at Washington Hospital Center is currently seeking a highly motivated Senior Acute Care Occupational Therapist who will be responsible for the initial evaluation, reevaluation, treatment planning, treatment implementation, reporting and documentation of all referred patients within departmental and professional standards. The Senior Acute Care Occupational Therapist serves as a resource in specified areas of expertise and participates in organizing educational activities and special projects, and facilitates staff development. In the absence of the Clinical Supervisor, the Senior Acute Care Occupational Therapist will provide administrative leadership as necessary.

Qualifications

EDUCATION: Graduate of an accredited school of Occupational Therapy with a Masters Degree in Occupational Therapy.

EXPERIENCE: At least three (3) years clinical experience in a rehabilitation or acute care setting required. Previous experience in teaching occupational therapy treatment principals and application to occupational therapy students and/or staff preferred.

LICENSE/CERT/REG: CPR Card; OccupationalTherapy license within the state of employment (DC/MD/VA).

SKILLS: Professional competency as a general practitioner in occuaptional therapy; demonstrates independent use of varied evaluation and treatment approaches.

Primary Duties and Responsibilities



•Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.
•Provides effective and efficient evidence based care following accreditation guidelines, licensing regulations, professional code of ethics, and standards of practice to include: evaluation/re-evaluation, treatment planning, DME recommendations and discharge planning. Provides education and training to patients and their families/caregivers. Accesses appropriate additional information for individualized patient care. Demonstrates sound clinical reasoning during patient care to maximize effective and safe outcomes.
•Completes accurate and timely documentation to include clinical interventions provided, discharge and equipment recommendations, charges, and prescriptions. Documentation will reflect the need for acute rehab level of care to include patient/family education, training, treatment intensity and duration. Documentation will follow established MedStar and departmental guidelines and will be comprehensive in nature and include conference reports, patient flow sheets, time off checklists and all other required supporting documents.
•Demonstrates effective time management and meets productivity expectations. Maintains accurate schedules for all patients and professional activities. Is timely, punctual, and is prepared for work schedule and meetings.
•Utilizes professional communication in decision making and problem solving. Consistently communicates and conducts self in a professional manner using appropriate written, verbal, and non-verbal communication methodologies. Participates in team and/or family conferences as appropriate. Collaborates interventions with other team members toward an interdisciplinary approach. Participates in general staff and team meetings. Communicates appropriate process improvements that impact patient care and operations.
•Participates in developmental activities for continued professional growth. Takes initiative in pursuing and directing continuing education for professional growth and competency for self. Serves as a resource and preceptor for new employee and student orientation for the Occupational Therapy Service and team based activities. Participates in development and implementation of goals and objectives for the program area as delegated by the Clinical Supervisor or management. Participates in peer review, program evaluation, and process improvement activities on a regular basis and for special projects. Assists in developing, revising or reviewing treatment guidelines in specialty area as delegated by the Clinical Supervisor or management. Serves as the Clinical Instructor for OT/COTA students and volunteers.
•Manages own time to meet productivity expectations and service demands and assists clinical supervisor with monitoring staff on team to assure adherence to quality improvement standards and budgetary guidelines. Attends appropriate service/hospital meetings. Completes assigned administrative duties to include data collection for statistics and reposts. Assures compliance of hospital and service policie sand procedures.
•Participates in multidisciplinary, quality and service improvement teams as appropriate. Participates in meetings, serves on committees and represents the department and hospital/facility in community outreach efforts as appropriate.
•Performs other duties as assigned.
