Job Responsibilities

As a Home Health Physical Therapist PT you will be responsible for planning and implementation of the primary care service delivery to patients per health care plan and written protocols. Provide primary, secondary and tertiary care, including counseling and referral within multidisciplinary team.
Additional responsibilities for the Home Health Physical Therapist PT include:

Monday through Friday 8am to 4:30pm
Saturday Rotation Schedule which will replace a weekday.
6-7 Visits Per Day
Performs comprehensive subjective and objective admission and ongoing assessment of client status that includes physical, psychosocial and environmental parameter.
Formulates individualized plan of care that incorporates analysis of assessment data and current scientific findings.
Provides skilled interventions aimed at achieving realistic outcomes within a specified time period.
Modifies and updates plan of care to reflect progress toward goals.



Job Requirements

We are looking for dedicated and professional Home Health Physical Therapist PT who are committed to providing exceptional patient care and service. It is also important that you are highly organized and that you demonstrate excellent communication and interpersonal skills.
 
Specific qualifications for the Home Health Physical Therapist PT role include:

Graduate of a program in physical therapy approved by the Council on Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association or its equivalent;
One year of professional physical therapy experience under the appropriate supervision; or
A combination of education and experience that demonstrates the person has acquired the necessary competencies required for the position.
Possession of a current State of New Hampshire license to practice as a registered physical therapist.
Must be able and willing to learn to utilize a portable electronic field device.  Must be able to manage electronic patient health information (ePHI) remote access in accordance with  policies and procedures.
Valid driver��s license and access to an automobile during working hours that meet the State of New Hampshire registration regulations and auto liability insurance.

Who will you treat? We are an acute rehabilitation hospital setting. Typically our therapist can expect to spend an average of 2-4 weeks with our patients treating a wide clinical caseload such as: stroke, spinal cord injury, brain injury, various neurological disorders, rheumatoid arthritis, osteoarthritis, amputation, joint replacement, fractures, multiple trauma, cardiac conditions, and pulmonary conditions. 80% of our patients are discharged directly home.

We use evidence-based practice guidelines to treat our patients. We utilize our clinical outcomes to assess and improve our treatment interventions and programs. We participate in a national outcomes database, which provides benchmarks and sophisticated outcome reports to help us continually analyze and improve our patient outcomes.

You will use standard and advanced therapy technologies- depending on hospital location, some equipment used such as Auto Ambulators, Bioness, Interactive Metronome, Hand Mentor, SmartStep, Anodyne, Wii and CAPs may be available.

The position is responsible for supporting and promoting the development and engagement of infants/toddlers and their families in everyday routines. Participate on the North Central Early Intervention Evaluation team and be responsible for determining the eligibility for infants/toddlers as related to motor and other areas of development. This position would also provide early intervention supports to the families of infants/toddle eligible for the program.

Participates in North Central Early Steps eligibility team evaluations and assists team with the completion of evaluations, assessments and in the completion for those eligible families an Individual Family Support Plan (IFSP). Provides adult learning and parental education as related to the IFSP outcomes and goals. Collaborates with multidisciplinary team in and around our ten county area. Supports and facilitates each child\\\'s progress through developmental milestones, increases opportunities for and enhances trusted caregiver-child relationships.

Conducts intervention sessions and participates in team consultation in the natural environment using a strength-based primary provider approach. This position will be responsible for ongoing assessment to determine if goals/outcomes are being met on the families IFSP. Training, special instruction and assistance technology needs for our families are vital parts of this position.

This position acts as a community liaison for the Early Steps Program, networking with other providers of early intervention services, including the public school systems, private providers, and the medical system. Providing guidance and consultation to our team of ITDS to ensure that their Plans of Care (POC\\\'s) meet with EI Medicaid guidelines. This position is required to participate in in-service training programs as specified by the State Early Steps Program. Completion of this requirement must be documented in the coordinator\\\'s personnel file.

Maintains and submits intervention notes and billing information timely and accurately for records and payment. Maintains clinical materials, supplies, and work area, and submits requests for replacement or repair as needed.
